payout cards

We need a payout card (paper) for a mills futurity. Original or quality reproduction.
Also, since the paint is chipping on the back glass, we need a bright payout card for a Jennings Standard Chief. Oval with fan at top & bottom, 3-5-11.
Does anyone remember Jack & Martha Connolly??? Super nice folks!!!
